Where time goes in a clinic

The clinical work is rarely the bottleneck. The front desk is.

Bookings, reminders, intake forms and insurance paperwork follow the same pattern every time and need no doctor, which makes them a good fit for automation.

In most Indian clinics we talk to, the receptionist is doing four jobs at once: answering calls, replying to WhatsApp messages, rescheduling patients and chasing payments. Every booking that arrives as a chat message has to be typed again into a register or a practice system. Reminders for the next visit are sent by hand, or not at all, so recall patients drift away and chairs sit empty.

Paperwork adds to it. New patients fill in a paper intake form that someone transcribes later. Lab reports arrive as PDFs on email or WhatsApp and have to be matched to the right patient. For cashless insurance, the clinic fills pre-authorisation forms for the TPA, attaches reports and follows up on the claim, often from a shared phone.

None of this needs a doctor, and most of it follows the same pattern every time. That makes it a good fit for healthcare automation: a booking flow on WhatsApp, document reading with a person checking the result, and reminders that go out on schedule. Our AI workflow automation service is where most clinic projects start.

What we build for clinics

Each piece works alone. Most clinics start with one and add the next once the first is trusted.

WhatsApp booking and reminders

Patients pick a slot on WhatsApp and get reminders and recall messages on schedule.

Patients message the clinic number, pick a slot from the live calendar and get a confirmation. Reminders go out the day before, and recall messages go out when a review or cleaning is due. Built on the WhatsApp Business API through our WhatsApp CRM, with every message template approved by Meta before use. If you do not have API access yet, we handle the WhatsApp Business API setup in an account the clinic owns.

Intake and report reading

OCR reads intake forms and lab reports, and staff confirm before anything is saved.

Read more about Intake and report reading

Scanned intake forms, referral letters and lab report PDFs are read by an OCR pipeline that pulls out name, phone, age, complaint and test values, then waits for a staff member to confirm before anything is saved. See OCR and document intelligence.

Enquiry pipeline

Instagram, Google and website enquiries in one Desk list, with the source recorded.

Enquiries from Instagram, Google and the website land in one list in Desk, with the source recorded, so the clinic can see which channel brings patients who actually book, and nobody's question is left unanswered.

Billing and GST

Invoices, expenses and bank matching in Fint, with GST charged only on taxable lines.

Invoices, expenses and bank matching in Fint. Useful for clinics that also sell taxable items such as products or cosmetic treatments, where GST has to be charged on some lines and not others.

Private AI for notes

Note summaries drafted by a model on the clinic's own machine, not an outside service.

Summaries of consultation notes or discharge instructions drafted by a language model that runs on the clinic's own machine, so patient text is not sent to an outside AI service. See private AI deployment.

Clinic workflows and what replaces them

The work that repeats every day, and the system that takes it over.

Nothing here makes a clinical decision. Every step that touches a diagnosis, prescription or payment has a named person who confirms it.

Rules a clinic system has to respect

We design around these from the first week, not as an afterthought.

Fig. 01 Overhead: messages filed into three pipeline lanes
Regulation and practiceIndia and UK
DPDP Act 2023
Patient data is personal data. Patients need a clear notice and must consent to its use. The DPDP Rules were notified in November 2025, and most duties apply 18 months after that.
Telemedicine Practice Guidelines 2020
Teleconsultations need patient consent and records of the consultation, and limit what may be prescribed remotely.
ABDM
Ayushman Bharat Digital Mission: ABHA health IDs and consent-based sharing of records between registered facilities.
Clinical establishment rules
Registration and record keeping under the Clinical Establishments Act 2010 in states that adopted it; Tamil Nadu has its own state Act for private clinical establishments.
Advertising
Medical council professional conduct rules restrict how doctors may advertise, so clinic social posts stay informational.
GST
Most healthcare services by a clinical establishment are exempt; cosmetic procedures and products sold are not.
UK GDPR
Health data is special category data and needs a lawful basis plus an Article 9 condition before it is processed.

Clients in healthcare

Let Smile Clinic is one of our healthcare clients. We work alongside your existing practice system instead of selling one, and can meet Chennai clinics in person.

Read more about Clients in healthcare

Let Smile Clinic is one of our healthcare clients. We are a Chennai team, so clinics in Chennai and across Tamil Nadu can meet us in person; we can also work remotely with clinics in other Indian cities, the UK and the Gulf.

We do not sell a practice management system. If you already run one, we connect to it where it has an API or an export, and where it does not, we build the booking and reminder flow alongside it with a daily sync your staff can check. Clinics where most bookings still come by phone can add a voice AI agent that answers routine calls and hands over to reception.

Questions

Can patients book appointments on WhatsApp without calling?

Yes. Using the official WhatsApp Business API, a patient can message the clinic, choose an open slot and receive a confirmation. The flow reads and writes the clinic calendar, and reception can still override it. Messages the clinic starts, such as reminders, use templates that Meta approves in advance, and patients must have opted in.

Does patient data leave India or go to an AI company?

It does not have to. For document reading and note summaries we can run open-weight models on a machine at the clinic or on a server you control, so patient text never goes to an outside AI service. Where a cloud service is used, we tell you which one and what it receives before anything goes live.

Will AI make clinical decisions?

No. We automate bookings, reminders, paperwork and data entry. Anything that touches diagnosis, prescriptions or what a patient is told about their health is reviewed by the doctor or a named staff member before it is saved or sent.

How does this fit with the DPDP Act?

We build consent capture into the first message, store only the fields the clinic needs, log who viewed or changed a record, and give you a way to delete a patient's data on request. Your own legal adviser should confirm your notice text; we make sure the system can do what the notice promises.

We are a single-doctor clinic. Is this too much for us?

Start small. A WhatsApp booking and reminder flow is often the only piece a single-chair dental or physiotherapy clinic needs, and it can run without any other system. Book a consultation and we will tell you whether it is worth doing.
